Medicaid Patients

LogistiCare provides transportation to and from medical appointments for Medicaid patients. There is no cost to the patient for this service. Call 804-236-1570. Patients will be asked to provide their Medicaid ID Number, the address at which they wish to be picked up and their destination address.
